The following form must be completed by a parent or guardian. Consequently, the first e-mail address requested must also be that of a parent/guardian. Forms completed by a student will NOT be accepted. Prior to completion, please review the following information regarding attendance in the CHS Instrumental Music Member Handbook.
SCHOOL Day
The school attendance office personnel should be contacted by the parent for all school day absences. The status of all absences is entered into school records by the attendance office. Keep in mind that class participation is included in the evaluation process of determining grades and daily class work is impacted if the student is not in class. Any absence considered truancy by the school will result in the student receiving a zero for all the work done that day.
PERFORMANCE
It is the student’s responsibility to make sure that proper notification has been made for any absence which occurs.
The student's parent or guardians must complete the Absence Request Form found on www.carrollbands.org prior to any anticipated absence from a performance to discuss the circumstances and possible options
In the case of an unplanned emergency, such as sickness, parents should contact Mr. Hassell or Mr. Frazier (in person or by telephone) prior to the absence. Text messages are not permitted.
Make-up work is required for any absence from a performance.
The student will not be allowed to participate until a parent/student conference is held if prior notification has not been made.
Attendance and full participation is expected at every performance, as long as the student is physically able to do so.
Unexcused absences from performances may result in loss of current and/or future leadership position, loss of eligibility for letter and patches, and loss of eligibility for participation in future after-school performing ensembles, including marching band, pep band, winter guard, indoor winter percussion, and all jazz ensembles.
REHEARSAL

The student will not be allowed to participate until speaking with a director
Students who feel ill during rehearsal may be allowed up to 15 minutes to sit out of rehearsal. If, after 15 minutes a student is unable to fully participate in rehearsal, they will be sent home to get rest and/or treatment. Students who are consistently unable to participate in rehearsals may not be permitted to perform until they can consistently and fully participate in rehearsals. Students absent from rehearsals are subject to a review by band staff before being eligible to perform in the next scheduled performance.
